Reporting and Assessments 

The Florida Department of Health in Hamilton County offers a wide range of services that focus on promoting and improving the health of residents and visitors of our community. Community Health Planning and Statistics services include continually assessing the health of the county and identifying areas for health improvement. Health assessment reports are prepared using a collaborative process to collect, analyze, and use data to inform and mobilize communities. These assessments contribute to a comprehensive view of health and quality of life in Hamilton County, and inform efforts to improve population health.

Community Health Improvement
